Booking and Appointment Scheduling

To maintain the highest standard of medical genetics, metabolic evaluations, and personalized precision healthcare at David Medical LLC, all consultations (in-person and telehealth sessions across licensed jurisdictions including Florida, California, and Colombia) must be scheduled in advance through our official channels: our online booking system, phone (+1 407-576-2576), or the Patient Portal (healow™ app practice code: BHAFDD). A valid credit card on file or deposit may be required at the time of scheduling.

Cancellation and Rescheduling Window (48-Hour Notice Required)

Specialized clinical evaluations, pre-test/post-test genetic counseling, and genomic testing reviews require significant medical preparation and dedicated clinical time.

• A minimum of 48 hours' advance notice prior to your scheduled consultation time is required to cancel or reschedule without incurring a financial penalty.
• Cancellations made with more than 48 hours' advance notice will allow any pre-paid fees or deposits to be fully credited toward a rescheduled appointment.

Late Cancellation and No-Show Charges

• Late Cancellations (less than 48 hours' notice): cancellations or modification requests made with less than 48 hours' notice are subject to a fee equal to 50% of the scheduled consultation or service fee, or forfeiture of the appointment deposit.
• Unnotified Absence (No-Show): if a patient fails to attend a scheduled in-person or virtual telehealth appointment without prior written or verbal notification, a 100% service charge of the scheduled consultation fee will be billed to the credit card on file or billed directly to the patient statement. Insurances do not cover No-Show fees.

Late Arrival Policy

We strive to see every patient on time. A maximum grace period of 15 minutes is allowed for late arrivals.

• Arriving within the 15-minute window may result in a modified or shortened consultation to avoid delaying subsequent medical appointments, while the full service fee will apply.
• Arriving more than 15 minutes late for an in-person or virtual consultation will require rescheduling and will be treated as a Late Cancellation under this policy.

Accepted Payment Methods and Insurance

David Medical LLC accepts the following payment methods:

• Major credit and debit cards (Visa, Mastercard, American Express, Discover).
• Electronic funds transfers and secure online portal payments via healow™.
• Cash directly at our Winter Park clinic office.
• In-Network Insurance Plans: United Healthcare, Cigna, Multiplan, Orlando VA (via UHC Optum), APWU Health Plan, Tricare East/Humana Military, Champ VA, Medicare Florida, Medicaid Florida, Aetna Better Health, Evolutions Healthcare, First Health Coventry, and Curative.
• Out-of-Network & Self-Pay: for out-of-network insurance plans or self-pay patients, affordable out-of-pocket pricing is provided. Co-pays, deductibles, and co-insurance are due at the time of service.

Service, Consultation, and Testing Refund Policy

• Clinical Consultations & Rendered Medical Services: all completed medical evaluations, physician consultations, genetic counseling sessions, and diagnostic interpretations conducted by our clinical team are non-refundable.
• Genomic & Laboratory Testing: laboratory fees for genetic panels, genome sequencing, and diagnostic kits ordered through third-party laboratories (e.g., Invitae, Variantyx) are non-refundable once the sample collection has been processed or submitted to the processing laboratory.
• Pre-Paid Packages or Care Plans: unused balances for pre-paid multi-step evaluations may be eligible for pro-rated account credits (Store Credit) upon clinical management approval if medical reasons prevent completion, minus any completed consultations or processed diagnostic testing.

Minors and Pediatric Patients

David Medical LLC provides comprehensive pediatric genetic consultations as well as adult precision medicine evaluations at our Winter Park location (1400 S Orlando Ave #105, Winter Park, FL 32789).

• Pediatric Patients: minors receiving clinical evaluations or genetic counseling must be accompanied at all times by a parent or legal guardian.
• Non-Patient Minors: to maintain a quiet, secure, and confidential medical environment for complex genetic consultations, unattended minors are not permitted in waiting areas or consultation rooms. If an adult patient arrives with a minor who requires supervision, an accompanying adult must be present, or the appointment may need to be rescheduled under our cancellation terms.

Service Animal and Pet Policy

• General Pets: for clinical hygiene, patient safety, and allergen control in our medical facility, domestic pets and emotional support animals are strictly prohibited inside the clinic premises.
• Service Animals: fully trained and certified Service Dogs assisting individuals with disabilities are welcome, in accordance with the Americans with Disabilities Act (ADA) and Florida state law. Service animals must remain under the handler's control (leashed or harnessed) at all times.
